Around 15% of adults live with tinnitus, a persistent ringing or buzzing with no external source and, for most, no adequate solution. For those with severe symptoms, the consequences extend well beyond discomfort: difficulty concentrating, broken sleep, anxiety, and depression that compound over years. Medications are widely sought, but have limited evidence of effectiveness. The intervention with the strongest evidence, cognitive behavioural therapy (CBT), has rarely been accessible to the people who need it most.
Like many clinicians working with tinnitus, Professor Vinaya Manchaiah saw the same painful pattern again and again. People were desperate for help, but the support that could actually reduce their distress was difficult to access. He began by delivering a CBT-based programme one-to-one with patients. It worked, but it could only reach the people in front of him. He wanted to make it available to millions.
Together with Professor Gerhard Andersson at Linköping University in Sweden and Dr Eldre Beukes at Anglia Ruskin University in the UK, Vinaya helped develop one of the world’s most extensively validated digital tinnitus programmes.
Across multiple randomised controlled trials, 60-70% of users experienced clinically meaningful improvement after completing the programme, and those gains held for at least a year. The team then replatformed the programme to Cogniss so it could move beyond the research setting and into patients’ hands.

Tackling Tinnitus is an eight-week, self-guided CBT programme delivered through a secure native mobile app, with flexible scheduling, relaxation and sleep strategies, progress worksheets, and optional hybrid check-ins with clinicians.
For someone living with severe tinnitus, relief is not a small thing. It can mean sleeping through the night, concentrating at work, feeling less anxious, and recovering a sense of control over a sound no one else can hear. Tackling Tinnitus has shown that this kind of relief is possible.
Yet despite its evidence base and affordability, getting Tackling Tinnitus into routine care has been challenging. For health providers, it is one more proven solution in a crowded field of targeted apps, each of which needs to be reviewed, integrated and implemented separately.
A proven app exists. Millions could benefit. The missing piece is a simpler path into care.

Prof Vinaya Manchaiah is an internationally recognised audiology researcher and clinician focused on improving support for people with hearing and balance conditions. He is Professor of Otolaryngology, Head & Neck Surgery at the University of Colorado School of Medicine, Director of Audiology at UCHealth, and Principal Investigator at the Virtual Hearing Lab.
His research focuses on making care more accessible, affordable and effective through self-management and digital technologies. Across his career, he has published more than 275 peer-reviewed manuscripts and six textbooks, with his tinnitus work recognised by the British Tinnitus Association.
